• USA Online Hockey Registration  Every Player must Register Online with USA Hockey before they can skate.  Prior year Registration is good through August 31st.
   

• Turners Registration Manual   Print & Complete Manual, Turn into Team Manager.

• Coaches Application  Print & Complete Application, Turn into Steve Shoda or fax to 260-447-3251
   

• Yearbook / Program Fundraiser Packet  80% goes directly to skaters ice payment.  20% goes directly to the printing costs of the Program.
   

• Coaches Replacement Card Form
